This exciting new post will work across two busy central Community Mental Health Teams in the heart of Manchester supporting the delivery of safe, effective patient care for individuals with severe and enduring Mental Health problems.
The role will include supporting the simplification of accessing team assessments and continuing to support the duty system and team zoning.
Embedding CPA in a meaningful and holistic way and working with GP's primary care and carers of service users.
Developing strong links with GP's and referrers , phone calls, attending surgeries, meeting outside agencies, running clinics and assessing , diagnosing, prescribing and managing care autonomously.
Overseeing the teams physical health agenda engaging with other agencies and third sector to support patients in there physical and mental health recovery. Providing clinical leadership and management as part of the MDT team in two CMHTs. Leading improvements in the quality of care and safety through service development, evaluation and audit.
You must have appropriate UK professional registration.
